Apply for a Degree/Certificate (Graduation)  

• Submit this form if you are currently enrolled in the last remaining courses required for your degree/certificate or if you have already completed the requirements. Degrees/certificates are NOT granted automatically. Degrees/certificates are NOT granted automatically. You must apply for them to earn them.
• Degrees/Certificates will be mailed out 8-10 weeks after the semester ends. It is critical that you have a current mailing address on file. You can update your mailing address via the MyVCCCD portal.

Authorization for Release of Information

• In accordance with the Family Educational Rights and Privacy Act of 1974, also known as FERPA or the Buckley Amendment, the confidentiality and privacy of the academic records of our students must be maintained. Student record information cannot be released to anyone without the student's written consent. The federal law applies to all students attending any college within the Ventura County Community College District, regardless of their age. 
• When a student turns 18 years old or enters a postsecondary institution at any age, the rights under FERPA transfer from the parent to the student.

• AB 540 - CA Nonresident Tuition Exemption Request

  • Use this petition if you do not qualify for a CA residency reclassification and satisfy the requirements of AB 540 such as attending a minimum of 3 years in a California high school and graduating from a California high school, and satisfy all the requirements outlined in the form.
  • You may only use this form if you are currently residing in California. This form does not apply to students living outside California taking Distance Education classes.
• California Veteran Tuition Exemption - VACA
  • For eligible Veterans and dependents who reside in California. Documentation is required to establish eligibility. Covered individuals must enroll in Moorpark, Oxnard and Ventura colleges (VCCCD) in order to qualify for this exemption. The covered individual must also be physically present in CA and must have served at least 90 days in active duty status (or in the case of dependents, the member must have served at least 90 days in active duty status).
  • The student must qualify for “covered individual” status as of the “residency determination date” applicable to the term in question as provided by California Code of Regulations, Title 5 Section 54002). If a student veteran is not VACA eligible at the start of the term and becomes VACA eligible during that term, the student does not qualify as a “covered individual” for that term. However, the student is eligible for “covered individual” status under the VACA act for the subsequent term.
• Residency Reclassification Request
  • Use this petition if you were accepted as a nonresident for tuition purposes and wish to request a residency reclassification.
  • You may qualify for a reclassification if you have been living in California for at least a year and a day prior to the start of the semester, are financial independent, and can provide documentation to show your intent to make California your home and your physical presence in the state.
  • All documents submitted must be dated over one year and one day prior to the start of the term.

• Course Repetition to Meet Legally Mandated Training
  • Students are permitted to take courses offered at the colleges of Ventura County Community College District under a one-and-done provision. This means the student who takes a course that is not specifically annotated as repeatable in the catalog and earns a grade of “C/P/CR” or better may not repeat the course. An exception is allowed under special circumstances.   
  • Students are permitted to repeat courses determined to meet legally mandated training requirements. Repetition of the course must be required as a condition of continued paid or volunteer employment. Such courses may be repeated for credit any number of times, with appropriate documentation, per Title 5, section 55000. The grade and units earned each time the course is attempted shall be included for purposes of calculating the grade point average. The Ventura County Community College District reserves the right to require the student to self-certify the course repetition is necessary to complete legally mandated training pursuant to this section. The student is also responsible to provide the college with appropriate documentation of the necessity to repeat the course.
     
• Course Repetion - Significant Change in Industry or Licensure Standard
  • Use this petition when you need to retake a course where there has been a change in industry or licensure standards that repetition of the course is necessary for employment or licensure.

Credit for Prior Learning 

The purpose of Credit for Prior Learning (CPL) is to recognize a student's experience(s) and knowledge base for which credit or
advanced standing has not been previously granted. Knowledge and skills acquired through experience alone or by earning industry-recognized credentials, evaluated subjectively by faculty with subject matter expertise.

Student Eligibility Criteria:
Students must be admitted to the college where the available CPL course is currently being offered and is in good academic standing.
Transcripts of all previous coursework must be on file to verify no college credit exists in more advanced subject-matter, and that no grade
was previously issued for the course. Students may petition for CPL once per course and may not be currently enrolled in the course or
have a record of previous course attempts including comparable courses at another district college.

Limitations:
CPL will only be applied to VCCCD degree programs. No more than 50% of units required for an associate degree (excluding the
Associate Degree for Transfer and UC Transfer Pathways degree, which have no limit) or Certificate of Achievement can be petitioned.
CPL does not count toward the required 12 units in residency for an associate degree. Other colleges and universities may not accept CPL
for transfer purposes, including but not limited to meeting CSU GE, IGETC, pre-major, major, and/or other requirements. Units granted
through the CPL are not counted toward eligibility for athletics, financial aid, veterans benefits, selective service deferment, or Social
Security benefits.

AP/IB/CLEP: Do not use this petition. Instead, submit your official scores to the Admissions and Records Office and schedule an appointment with a counselor to discuss how these will be applied to the VCCCD degree and/or certificate.

HS Articulation: Do not use this petition. Instead, contact the lead college counselor or staff member for high school articulation to confirm completion of articulated coursework and eligibility for college credit, and the credit will be applied to the degree and/or certificate.

Veterans: If you are requesting CPL credit to meet the VCCCD GE Area E or CSU GE Area E only, do not complete this form. Instead, submit the official JST or acceptable document to Admission & Records Office to receive credit. Students requesting course credit beyond the HED and PE units will need to complete this form.

    • Note: Please double-check the email inputted to ensure it is correct. Incorrect emails will prevent the form from being routed for approval. 
  • Forms are sent electronically to A&R after the counselor and parent/guardian sign the form. 
    • Note: The parent/guardian's email and approval is only required once. If you previously submitted a form for approval, you will not be required to obtain approval again.

• Students 8^th grade or lower are required to obtain approval every semester from the VP of Academic Affairs or their designee to clear the student hold. This hold is essential to facilitate a smooth transition into Oxnard College's academic environment.
  • After submitting the dual enrollment form to Oxnard College, please email lgonzalez@vcccd.edu to request an appointment with the VP of Academic Affairs or designee. When sending your request from your MyVCCCD email, kindly provide your full name, student ID (900#), and any pertinent documents that may be necessary for the meeting.
  • The meeting will be an opportunity to talk about your academic journey, address any questions or concerns you may have, and make sure that all necessary steps are taken to create a successful educational experience at Oxnard College.
  • The VP of Academic Affairs, or their designee will determine if the student has the abilities and sufficient preparation to benefit from instruction at a community college, and that the student's safety and that of others will not be affected. The decision of the VP Affairs, or their designee shall be final.

If you have already received your original diploma and need additional copies, or if you lost your original diploma and need a replacement, complete the Duplicate Diploma Form. 

There is $15 fee per duplicate/replacement diploma. 

Students who graduated in the most recent term and have never received a diploma should check their MyVCCCD email with instructions for claiming their first complimentary diploma.

Health Fee Exemption Request

• Students enrolled in credit classes in the District are assessed a health fee which enables the student to utilize Student Health Services. The services provided are articulated in Education Code Sections 76401 and 76355; Title 5 Section 53411; Title 17 Section 2500, providing health services to assist students in maintaining physical and mental well-being so that they can pursue their educational goals. Services include medical, nurse, and mental health visits (individual and support groups).

• In accordance with California Education Code 76355, the fee will be charged to each student regardless of the number of units taken, including on campus and online courses. This fee is not waived by the California College Promise Grant. It is your responsibility to pay the health fee every semester. As specified in AP 5200, students who meet specific conditions may be exempted from paying the fee. Students approved under this exemption will not be eligible to receive any services at the Student Health Center except in the case of an emergency.

Request for Work in Progress

• Registration is normally prohibited where a course is currently in progress. This record allows intervention to permit registration.  Submit this form to your instructors to obtain the grade(s) to date in an effort to re-enroll in the same course for the following terms.

• Pass/No Pass Request

• Use this petition if you do not want to receive a letter grade in a class, but instead, receive a Pass or No Pass grade on your transcript. 
  • The student may elect the pass/no pass option for available courses until the last day of instruction of the term, whether the basis of evaluations is to be Pass/No Pass or a letter grade. Once the last day of instruction of the term has passed, the request cannot be withdrawn. It is the student's responsibility to file the appropriate form by the deadline; otherwise, a letter grade will be assigned.
  • Some courses are not available for the Pass / No Pass grading option and may not be petitioned. 
  • The deadline to apply for Pass/No Pass is the last day of the semester. 
• It is strongly suggested you speak with an academic counselor, at (805) 678-5816, before opting into Pass / No Pass for a course.

• Petition to Audit Class
  • Students enrolled in 10 units or more may, with instructor approval, be allowed to audit one lecture-only class per term without a fee. Students enrolled in less than ten units may, with instructor approval, audit one lecture-only class per term for a non-refundable fee of fifteen dollars ($15.00) per unit and must pay the student health fee. Priority for enrollment is given to students taking the course for credit towards a certificate or degree. Therefore, students wishing to audit may register for classes in audit status by petition only in the last two days of the add/drop period.
  • Laboratory and activity classes are not normally available for audit. Students are not permitted to change enrollment to receive credit for the course. Students auditing a course are not permitted to take exams or challenge the course at a later date. Instructors are under no obligation to grade assignments of students auditing a course. Attendance requirements for students auditing courses are the same as for all other students.
  • Audit fees are non-refundable. 

• Late Add Request 
  • Use this petition when you have been attending a class prior to the last day to add, but extenuating circumstances prevented you from registering.
  • Please contact your instructor directly for the completion of this form. If your Instructor authorizes you to take their class, they will complete this form in their faculty portal, under OC Faculty Forms. 

• The EW symbol may be requested by the student and used to denote excused withdrawal. Excused Withdrawal (EW) shall not be counted in progress notice or dismissal, nor shall it be counted towards the permitted number of withdrawals or counted as an enrollment attempt.
• An EW is acceptable when a student withdraws from a course due to extenuating circumstances beyond their control. An excused withdrawal shall not be counted in progress notice and dismissal calculations or toward the permitted number of withdrawals or enrollment attempts.
• Grade changes may be requested by the student no more than three (3) years after the term in which the grade was awarded, per VCCCD AP 4231.
• A change of academic grade to Excused Withdrawal (EW) will be recorded only after this petition has been completed, determined to meet the requirements of the policies of the VCCCD, and approved.

Petition for Military Withdrawal

• Use this petition to withdraw from your courses because you are a member of an active or reserve United States military service and received orders compelling withdrawal from courses.
  • If approved, a notation of MW will appear on your transcript instead of a letter grade or W and a refund will be issued.
• When you sign this form, you will be prompted to enter your email address to verify your identity. Only enter your MyVCCCD email.

Prerequisite Clearance Request

• If you are attempting to enroll in a course with an error reading, “See Counseling for Prerequisite Clearance” appears, or if you know you need to be cleared for a prerequisite or co-requisite, you must complete the online Prerequisite Approval Form before attempting to complete the course enrollment process again.

Student parents are now eligible for priority registration at Oxnard College. Starting on the first day of the priority registration period, student parents can apply for classes in upcoming semesters. 

What is the benefit of priority registration? Priority registration allows students to register early for classes, increasing the odds of getting the desired schedule.

What is a student parent? This is a student who has a child under 18 years of age receiving more than half of their support from their student parent. To be eligible, students must participate in orientation, complete the assessment process, and develop a Student Education Plan with an academic counselor. Continuing students must be in "good academic standing" (i.e., a student show is not on academic or progress standing for two semesters) or have completed fewer than 90 units.  

How do I apply if I am a student parent? Just submit our Priority Registration for Students Parents request form. These get forwarded to the Admissions and Records Office so your priority registration can be processed.

Reinstatement Petition 

• Use this petition when you have accidentally been dropped from a class you were previously enrolled.
• Please contact your instructor directly for the completion of this form. If your Instructor authorizes you to be in their class, they will complete this form in their faculty portal, under OC Faculty Forms.

Teaching Credential Candidate: Basic Skills Requirement Met by Coursework 

• According to AB 130 “New Basic Skills Requirement Options…the Commission and Commission-approved preparation programs [can] verify that a candidate or applicant has demonstrated basic skills proficiency by verifying qualifying college-level coursework from a regionally accredited institution of higher education if certain conditions are met."
• This letter, along with the student's Official Transcripts, serves as verification that the student has met the basic skills proficiency requirement by earning a grade of “B” or higher in one or more of the courses listed on the letter.

• Students can view their semester grades and unofficial transcript online via their MyVCCCD portal.

Transcript Additions

• Students may add up to five college recognitions to their transcript. These additions will appear on the student’s existing Oxnard College official transcript. Students may submit this form upon completion of their term of office in an organization.

• Term of office constitutes one complete semester in the organization and position held. If the advisor is no longer employed at Oxnard College to verify the term dates, the student is required to submit documented proof of their organization involvement and position held.  Please attach documentation to this form.  

• Complete and submit this form to the Student Activities Specialist, room #S-115 at the Student Activities Office.

• Please allow two weeks for the Admissions and Records Office to process your request.
